Please Note: When you are looking for an appointment, we are often booking out 4+ weeks. If you would like to be seen sooner, please make a note and we will add you to the waitlist and text you if something becomes available.

After you submit your request, you will receive a confirmation email. You will also receive an email to sign up for the online portal. This is a necessary step so that you can receive important information and documents. This must be completed at least 3 days prior to your appointment.

Unfortunately, due to federal law, we are unable to see patients with Medicare or Medicaid insurance - even if you plan on paying out-of-pocket.
